How we look after your information

Fat & Sen Disability Pty Ltd, trading as FSD Care, follows the Privacy Act 1988 (Cth), the Australian Privacy Principles and the Health Records Act 2001 (Vic). This policy explains what we collect, why, and what you can do about it.

What this website collects

Enquiries. When you use our contact form, we send your name, contact details, suburb and message to our office by email so we can reply. The website itself does not keep a copy.
FSD Care Hub updates. If you register interest in FSD Care Hub, we use your name, email and any details you add only to tell you about it. Ask us at any time and we will stop.
No tracking. This website does not use advertising or analytics cookies, and does not track you across other websites.
Security records. Our hosting provider, Cloudflare, processes your IP address and browser details briefly to protect the site from attacks and spam. These records are not used to identify you.

Information we collect when we support you

To provide safe, good-quality supports we collect information such as your name, contact details, date of birth, NDIS number and plan details, health and medication information, emergency contacts and the goals you want to work on. We collect it from you or, with your consent, from people such as your family, support coordinator, plan manager or doctor.

We use it to:

plan, deliver and review your supports
keep you and our workers safe
claim payment from the NDIA, your plan manager or you
meet our legal duties, including NDIS record-keeping and incident reporting
handle feedback and complaints

Who we share it with

We only share your information with your consent, or where the law requires or allows it, for example with the NDIA, the NDIS Quality and Safeguards Commission, or emergency services when someone is at serious risk. We never sell your information.

How we keep it safe

Participant records are kept in our secure client system, available only to staff who need them, with sign-in protected by two-step verification. We keep records for at least seven years as the NDIS rules require, then destroy them securely.

Seeing or correcting your information

You can ask to see the information we hold about you, or ask us to correct it, at any time. Contact us on (03) 4232 2503 or careteam@fatsendisability.com. We aim to respond within 2 working days, and always within 30 days.

Privacy complaints

If you are unhappy with how we have handled your information, please tell us first through our feedback and complaints process. If you are still not satisfied, you can contact the Office of the Australian Information Commissioner at oaic.gov.au or on 1300 363 992.
